PAYMENT POLICY

At Four Peaks Chiropractic we can bill most insurance plans directly, this coverage is for you a spouse and your dependents. Some insurance plans will allow us to directly bill, others will require copays and deductibles which will need to be paid first. At your first appointment if you have medical insurance please bring a copy of your card and plan so our office staff can assist you. We also work with auto accidents, personal injury and worker's compensation claims. If you have any questions about your insurance plan please feel free to contact our office for more information.

If you do not have insurance, or if your insurance does not cover chiropractic treatments, we have very affordable same day treatment discounts.

HEALTH PLANS

We are In-Network for Blue Cross Blue Shield, Aetna and Cigna.

 

We are Out of Network for United Healthcare and Banner Health.

We will be happy to contact your insurance company on your behalf to verify coverage and the terms of said coverage. If for any reason your coverage is insufficient, we have other affordable payment options we can discuss with you.

We also accept Medicare.
